PARTICIPATION WAIVER, LIABILITY RELEASE AND MEDICAL CONSENT

Voluntary Participation

Participation in sports activities organized by Shooting Star Sports Services LLC is voluntary and optional. Parents or guardians may decide whether their child participates in training sessions, camps, tournaments, travel programs, or other academy activities. Parents are responsible for determining whether participation is appropriate and safe for their child.

Assumption of Risk

The parent or legal guardian understands and acknowledges that participation in sports and physical activities involves inherent risks. These risks may include but are not limited to:

• Physical injury or serious bodily injury
• Permanent disability
• Illness or medical complications
• Property damage
• Loss of life or fatal accidents

Such risks may arise from physical activity, collisions with other participants, equipment use, environmental conditions, travel, or circumstances beyond the control of the academy. By allowing participation, the parent or legal guardian voluntarily accepts and assumes all risks.

Health and Medical Responsibility

Parents confirm that their child is physically capable of participating in sports activities. Parents must inform the academy of any medical conditions, allergies, injuries, or medications that may affect participation. Parents agree not to allow their child to participate in activities if the child is unwell, injured, or medically unfit.

Medical Authorization

In the event of injury or medical emergency, the parent authorizes Shooting Star Sports Services LLC and its staff to provide first aid and to arrange emergency medical assistance if required. This may include contacting emergency services or arranging transport to a hospital. Parents remain responsible for any medical expenses arising from such treatment.

Emergency and Safety Procedures

During activities, emergency situations may occur including government safety alerts, civil defense warnings, or security advisories. The academy may implement procedures including evacuation, shelter protocols, or suspension of activities. Participants and parents must follow instructions provided by academy staff or relevant authorities.

Venue and Facility Use

Activities may take place at schools or third-party venues. While the academy provides instruction and supervision during official hours, it does not control all aspects of facility operations. Participants must follow all safety rules of the venues used.

Independent Arrival and Departure

Parents may allow their child to arrive at or leave academy activities independently (walking, cycling, etc.). Academy supervision applies only during official program hours. Once a child leaves the program area or arrives independently, responsibility transfers fully to the parent or guardian.

Local Events and Competitions

Participants may take part in local matches, leagues, and tournaments. Parents acknowledge that competitive sports involve additional risks, including travel to venues and physical competition.

International Travel

The academy may facilitate international sports trips. Parents acknowledge that international travel involves additional risks including air travel, accommodation, and compliance with foreign laws. Parents are responsible for valid travel documents, visas, and insurance. Participation is strictly voluntary.

Government Directives and Regional Safety

Activities may be affected by government safety directives or regional security situations. The academy may suspend, postpone, or modify activities for safety. Parents remain responsible for deciding whether their child participates during such periods.

Media Consent

Photographs or videos may be taken for coaching review, communication, and promotional purposes. Parents grant permission for their child to appear in such materials unless written notice requesting exclusion is provided.

Limitation of Liability

To the maximum extent permitted under the laws of the United Arab Emirates, Shooting Star Sports Services LLC, its directors, employees, and representatives shall not be held liable for any injury, illness, damage, loss, or death arising from participation except in cases of proven gross negligence. Participation is at the participant’s own risk.
